2009 FCSS Applications

The
Family and Community Support Services Grant Program is a
source of funding provided by Alberta Children's Services AND
the City of Lloydminster. The funds are to be used to enhance
and enrich preventative programs and services within the City of Lloydminster.
Who May
Apply for Funding?
Applicants
must meet the criteria as set out in the Provincial Conditional
Agreement Regulations that provide direction for program
funding. All applicants must be incorporated under the
Societies Act or be a registered charity.
To be
eligible for funding, the program must:

promote,
encourage and facilitate the involvement of volunteers,
- encourage
efficient and effective use of resources,
- encourage
and facilitate cooperation and coordination with allied services and
agencies operating within the City of Lloydminster,
- promote,
encourage and facilitate the development of stronger communities,
- promote
citizen participation in planning, delivery, and the governance of
the program and of services provided under the program.
In
addition, services provided under an FCSS program must:

Be of a
preventative nature that enhances the social well-being of
individuals and families through promotion or intervention strategies
provided at the earliest opportunity; and

Do one of more
of the following:
- Help
people develop an awareness of social needs;
- Help
people to develop interpersonal and group skills which enhance
constructive relationships among people;
- Help
people and communities to assume responsibility for decisions and
actions which affect them;
- Provide
supports that help people as active participants in the community
Services
provided under an FCSS program MUST NOT:
***NEW REQUIREMENT***
Several years
ago, Family and Community Support Services undertook a project to
allow them to collect data and report outcomes from programs that
were supported. This project, known as MAD (Making a
Difference) requires data collection and reporting from the various
agencies. Part of this process requires agencies to develop a
Program Logic Model, which in turn will assist the agencies to
collect data and ultimately provide program outcomes. ALL AGENCIES
THAT RECEIVE FUNDING FOR 2008 PROGRAMS WILL BE REQUIRED TO
ENTER A PROGRAM LOGIC MODEL IN THE HOMES DATABASE. A sample
completed Program Logic Model has been included in the application
package. Once your program is completed, each program will be
required to enter "outcomes" in the HOMES database - this
process will need to be completed prior to final cheques being processed.
